Pharmacological properties
Daston is a drug that improves the metabolism of the myocardium and muscle tissue.
Pharmacodynamics
Phosphocreatine plays an important role in the energy mechanism of muscle contraction. The chemical structure is similar to macroergic endogenous phosphocreatine. It inhibits the process of destruction of the sarcolemma of ischemic cardiomyocytes and myocytes, provides intracellular energy transport. By improving microcirculation, the drug reduces the size of the zone of necrosis and ischemia. In ischemia and postischemic reperfusion, it has an antiarrhythmic effect, which is associated with a decrease in the ectopic activity of the ventricles and the preservation of the physiological function of Purkinje fiber cells.
Pharmacokinetics
With a single intravenous infusion, the maximum concentration of the drug in the blood plasma is determined for 1-3 minutes. The largest amount of the drug accumulates in the skeletal muscles, myocardium and brain. In the tissues of the liver and lungs, the accumulation of the drug is insignificant.
The half-life in the fast phase is 30-35 minutes. The half-life in the slow phase is several hours. Excreted by the kidneys.
Indications for use
In complex therapy:

· in sports medicine: to prevent the development of the syndrome of physical overstrain and improve adaptation to extreme physical exertion.
Contraindications

chronic renal failure (for use in doses of 5-10 g / day);

Dosage and administration
In acute myocardial infarction , on the 1st day, 2-4 g is injected intravenously, followed by a drip infusion of 8-16 g in 200 ml of 5% glucose solution for 2 hours. On the 2nd day: 2-4 g in / in drip 2 times / day. Day 3: 2 g intravenously 2 times a day. If necessary, a course of infusions of the drug 2 g 2 times / day can be carried out for 6 days. In chronic heart failure, it is administered intravenously at a dose of 1-2 g 2 times / day for 10-14 days.
In case of intraoperative myocardial ischemia, the drug is administered as part of a conventional cardioplegic solution in an amount of 3 g per 1 liter. Duston is added to the composition of the solution immediately before administration. It is recommended to conduct a course of infusions of the drug 2 g 2 times / day for 3-5 days before surgery, and within 1-2 days after surgery.
If it is possible to develop intraoperative ischemia of the extremities , it is recommended to inject 2-4 g of the drug intravenously before surgery, followed by an infusion of 8-10 g in 5% glucose solution during the operation and the reperfusion period.
Special instructions and precautions
Rapid intravenous administration of Daston in high doses (more than 1 g) can cause a drop in blood pressure.
The introduction of the drug in high doses (5-10 g / day) can lead to an increase in the amount of phosphates with a potential effect on calcium metabolism and on the secretion of hormones that regulate homeostasis, kidney function, and purine metabolism. Such doses should only be used in certain rare cases and for short periods.
